Effectiveness of Topical Antibiotics as Adjunctive Therapy in Periodontitis

Aggressive periodontitis is a rapidly progressing periodontal inflammatory condition that can lead to significant damage to the tooth-supporting tissues. Non-surgical therapy such as curettage is often associated with the use of systemic or local antibiotics to improve clinical outcomes.

A study conducted by Aag Bayu Apri Buana, a student of the Faculty of Dentistry, Universitas Gadjah Mada, under the supervision of drg. Kwartarini Murdiastuti, Ph.D., Sp.Perio(K), entitled “The Effectiveness of Combined Topical and Oral Minocycline Administration in Curettage Treatment of Aggressive Periodontitis”, examined the clinical effectiveness of using minocycline as adjunctive therapy.

  • The study involved 32 periodontal pocket sites in patients with aggressive periodontitis, divided into two groups:
    • Curettage + oral minocycline + topical minocycline gel
    • Curettage + oral minocycline only 
  • Measurements were taken at baseline, 1 month, and 3 months, using:
    • Probing Depth (PD)
    • Relative Attachment Level (RAL)
    • Alveolar bone loss 
  • Key Findings:
    • Both treatment groups showed improvement in PD reduction and RAL gain compared to baseline.
    • The combination of topical and oral minocycline provided significantly greater PD reduction and RAL improvement at the third month compared to oral therapy alone.
    • No significant difference was found in the reduction of alveolar bone loss between the two groups.

    • The use of topical minocycline gel as adjunctive therapy strengthens the effect of curettage in reducing inflammation and supporting tissue regeneration.

    • This combination therapy is particularly relevant for achieving effective non-invasive management in patients with aggressive periodontitis.

In conclusion, the adjunctive use of topical minocycline together with oral minocycline provides significantly superior clinical improvements in probing depth reduction and attachment gain compared to oral therapy alone. Although alveolar bone improvement was not significantly affected, this approach demonstrates strong clinical value in managing aggressive periodontitis.
