Within seconds, X-rays sweep across the entire jaw. The process is painless and invisible. Yet beneath the surface, gingival mucosal cells absorb radiation that can trigger oxidative reactions, damage DNA, and induce local inflammation.<\/strong><\/p>\n\n\n\n<p class=\"wp-block-paragraph translation-block\">A study published in AIP Conference Proceedings (2019) by a team from the Faculty of Dentistry, Universitas Gadjah Mada (UGM), reported a rather surprising finding: a beta-carotene-based gingival mucoadhesive patch was able to suppress two markers of biological damage simultaneously\u2014gingival crevicular fluid (GCF) volume and 8-oxo-dG levels\u2014following panoramic radiographic exposure. The study included drg. Rurie Ratna Shantiningsih as the corresponding author, along with Silviana Farrah Diba and drg. Anggun Dwi Andini, Sp.KGA.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>When Dental X-Rays Leave Their Mark on Tissue<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">GCF is an exudative fluid secreted by the gingival mucosa through the gingival sulcus. Under normal conditions, it is present in small amounts. However, radiographic exposure can increase the permeability of capillaries within the gingival plexus, resulting in increased GCF production. This is not merely a change in fluid volume. An increase in GCF is also a marker of an inflammatory process.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">X-ray radiation can also trigger the formation of 8-oxo-dG, a DNA adduct produced through oxidative reactions. This compound has long been recognized as a biomarker of DNA damage caused by reactive oxygen species (ROS). The higher its concentration, the greater the genetic damage that has occurred in mucosal cells.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This is where beta-carotene comes into play. This carotenoid compound is known as a potent antioxidant capable of scavenging singlet oxygen and inhibiting ROS accumulation. It has also been shown to suppress NF-\u03baB activation in stimulated immune cells, thereby reducing the expression of pro-inflammatory genes.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>A Small Patch with a Dual Mechanism<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">In this study, 20 subjects requiring panoramic radiographic examinations were divided into two groups. The experimental group received a beta-carotene mucoadhesive patch applied to the labial gingival mucosa of the maxillary central incisors (#11 and #21) before exposure. The control group received no treatment.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Five minutes after exposure, GCF was collected using filter paper inserted into the labial gingival sulcus for one minute. The volume was measured using 2% ninhydrin staining, which turns the wetted area purple. 8-oxo-dG levels were analyzed using the Enzyme-Linked Immunosorbent Assay (ELISA) method at a wavelength of 450 nm.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The results were clear. The group using the beta-carotene patch had significantly lower GCF volumes than the control group (p &lt; 0.05). For 8-oxo-dG levels, the experimental group recorded a mean of 0.767 \u00b1 0.368, while the control group reached 1.082 \u00b1 0.547, with the difference also being statistically significant (p = 0.036).<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>\u201cApplication of a beta-carotene mucoadhesive gingival patch can prevent increases in GCF volume and 8-oxo-dG levels following panoramic radiographic exposure of the gingival mucosa.\u201d<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This was the conclusion reached by the researchers in their paper.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>From the ALARA Principle to Clinical Application<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph translation-block\">Dental radiology has long adhered to the ALARA principle, which stands for As Low As Reasonably Achievable. In other words, radiation exposure should be minimized as much as reasonably possible. Traditionally, protective measures have focused on the use of lead aprons and beam collimation. However, the gingival mucosa, which lies directly within the exposure field, has rarely received additional protection.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">The mucoadhesive patch works on two levels. Physically, it coats the gingival mucosa and limits direct interactions between singlet oxygen and the tissue. Chemically, beta-carotene enhances gap-junction communication between cells by increasing connexin-43 expression, which contributes to maintaining tissue integrity.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">This study builds on a series of previous investigations, including an in vivo study in New Zealand rabbits demonstrating that a similar patch could prevent the formation of DNA adducts. In an earlier human clinical study, a reduction in the number of micronuclei in the gingival mucosa following panoramic radiography had already been observed, although the reduction did not reach statistical significance. With two new parameters\u2014GCF volume and 8-oxo-dG levels\u2014the evidence supporting the patch's efficacy has become stronger.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Potential Awaiting Further Development<\/strong><\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">A reasonable question arises: could this patch one day become standard equipment in dental radiology rooms, much like the lead apron is today? This study does not directly answer that question. The small sample size, with only 10 subjects per group, was acknowledged by the researchers as a limitation. Larger studies incorporating additional variables are still needed.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Nevertheless, the direction of this research is clear. Researchers at the Faculty of Dentistry, UGM, are testing the ability of a small patch made from a natural compound to protect tissues that have traditionally received little attention.
