{"id":20493,"date":"2026-07-16T15:09:38","date_gmt":"2026-07-16T08:09:38","guid":{"rendered":"https:\/\/fkg.ugm.ac.id\/?p=20493"},"modified":"2026-07-16T15:09:40","modified_gmt":"2026-07-16T08:09:40","slug":"racun-bakteri-dari-mulut-yang-mengacak-acak-sel-hati","status":"publish","type":"post","link":"https:\/\/fkg.ugm.ac.id\/en\/racun-bakteri-dari-mulut-yang-mengacak-acak-sel-hati\/","title":{"rendered":"Racun Bakteri dari Mulut yang Mengacak-acak Sel Hati"},"content":{"rendered":"\n<p><strong>Bayangkan sebuah bakteri yang selama ini berdiam tenang di rongga mulut, tiba-tiba berubah menjadi ancaman mematikan bagi hati dan otak. Itulah <\/strong><strong><em>Streptococcus intermedius<\/em><\/strong><strong>, dan penelitian Prof. drg. Heni Susilowati, M.Kes., Ph.D., PBO bersama tim dari Universitas Tokushima, Jepang, membuktikan bagaimana racun yang dihasilkan bakteri ini mampu memicu serangkaian reaksi molekuler berbahaya di dalam sel saluran empedu manusia.<\/strong><\/p>\n\n\n\n<p>Hasil penelitian ini dipublikasikan dalam jurnal <em>Biochemical and Biophysical Research Communications<\/em> (2011) dan menjawab pertanyaan yang selama ini menggantung: apa yang sebenarnya terjadi di tingkat sel ketika intermedilysin (ILY), toksin utama <em>S. intermedius<\/em>, menyerang sel-sel intrahepatic bile duct?<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Ketika Kalsium Berubah Menjadi Sinyal Bahaya<\/strong><\/h2>\n\n\n\n<p>Di laboratorium Oral Microbiology Universitas Tokushima, Prof. Heni dan tim menggunakan sel HuCCT1, yakni lini sel karsinoma kolangioseluler manusia, sebagai model. Sel-sel ini dipilih bukan tanpa alasan: saluran empedu intrahepatik mengekspresikan protein CD59 dalam jumlah tinggi, dan ILY diketahui mengikat sel inang justru melalui protein tersebut.<\/p>\n\n\n\n<p>Dengan konsentrasi ILY hanya 10 ng\/ml, tim peneliti mengamati sesuatu yang belum pernah dilaporkan sebelumnya. Dalam lima menit pertama setelah paparan, kalsium bebas intraseluler ([Ca\u00b2\u207a]i) tidak hanya meningkat, tetapi terakumulasi secara spesifik di nukleus sel. Fenomena ini divisualisasikan menggunakan probe Fluo-4\/AM di bawah mikroskop konfokus, dan hasilnya gamblang: nukleus sel menyala hijau terang, tanda kalsium membanjiri inti sel.<\/p>\n\n\n\n<p>Kalsium yang membanjir itu bukan sekadar penonton. Ia segera mengaktifkan calcineurin, sebuah enzim fosfatase yang kemudian mendephosphorilasi NFAT1, faktor transkripsi kunci dalam respons imun dan inflamasi. NFAT1 yang terdephosphorilasi lantas bermigrasi masuk ke nukleus, siap mengaktifkan gen-gen target.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Selektif dan Spesifik: Hanya NFAT1, Bukan NF-\u03baB<\/strong><\/h2>\n\n\n\n<p>Salah satu temuan paling menarik dalam penelitian ini adalah soal selektivitas. Pada sel hepatik HepG2, ILY memang mengaktifkan NF-\u03baB, faktor transkripsi yang lazim dikenal sebagai &#8220;sakelar&#8221; peradangan. Namun pada sel HuCCT1 yang berasal dari saluran empedu, ceritanya berbeda.<\/p>\n\n\n\n<p><em>&#8220;ILY tidak memengaruhi lokalisasi NF-\u03baB di sel HuCCT1, meskipun toksin yang sama mengaktifkan NF-\u03baB di sel HepG2. Ini mengindikasikan bahwa NFAT1, bukan NF-\u03baB, lebih sensitif terhadap ILY pada sel saluran empedu.&#8221;<\/em><\/p>\n\n\n\n<p>Temuan ini menjelaskan mengapa <em>S. intermedius<\/em> secara selektif merusak sel saluran empedu pada kasus abses hati. Bukan karena semua sel bereaksi sama, melainkan karena setiap jenis sel memiliki &#8220;bahasa&#8221; sinyal yang berbeda dalam merespons serangan bakteri.<\/p>\n\n\n\n<p>Untuk memastikan jalur calcineurin\/NFAT memang berperan, tim menggunakan dua inhibitor calcineurin yang sudah dikenal luas dalam dunia medis: cyclosporine A (CsA) dan tacrolimus (FK506). Hasilnya tegas. Kedua obat imunosupresan tersebut berhasil menghambat sepenuhnya translokasi nukleus NFAT1 yang dipicu ILY. Begitu pula dengan BAPTA\/AM, chelator kalsium intraseluler, yang menekan dephosphorilasi NFAT1 meski tidak secara total.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>EGR-1: Pemain Tersembunyi di Balik Kerusakan Hati<\/strong><\/h2>\n\n\n\n<p>Penelitian ini tidak berhenti pada NFAT1. Melalui analisis microarray, tim menemukan bahwa ILY juga menginduksi ekspresi <em>early growth response-1<\/em> (EGR-1), faktor transkripsi yang terlibat dalam pertumbuhan sel, proliferasi, dan respons imun.<\/p>\n\n\n\n<p>Lebih jauh, ketika sel dipra-perlakukan dengan CsA sebelum dipapar ILY, ekspresi mRNA EGR-1 yang diukur melalui real-time PCR turun drastis. Ini membuktikan bahwa ekspresi EGR-1 yang dipicu ILY berjalan melalui jalur calcineurin\/NFAT, bukan melalui mekanisme lain yang independen.<\/p>\n\n\n\n<p>EGR-1 diketahui berinteraksi secara fisik dengan NFAT1 dan bersama-sama menginduksi ekspresi sitokin proinflamasi seperti IL-2 dan TNF-\u03b1. Dalam konteks abses hati, kolaborasi antara NFAT1 dan EGR-1 yang teraktivasi oleh ILY ini diduga menjadi bagian dari mekanisme kerusakan jaringan yang progresif.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><strong>Dari Rongga Mulut ke Inti Sel Hati<\/strong><\/h2>\n\n\n\n<p>Penelitian ini membuka perspektif baru tentang bagaimana bakteri komensal rongga mulut, yang tampak tidak berbahaya dalam kondisi normal, bisa berubah menjadi agen patogen sistemik yang serius. <em>S. intermedius<\/em> dengan strain penghasil ILY tinggi diketahui erat berkaitan dengan abses hati dan otak, dan kini mekanisme molekulernya mulai terkuak.<\/p>\n\n\n\n<p>Bagi dunia kedokteran gigi, temuan Prof. drg. Heni Susilowati dan tim ini menegaskan sesuatu yang kerap terlupakan: kesehatan rongga mulut bukan urusan lokal.
