Di dalam alat itu, tertanam dua keping kecil resin komposit \u2014 bahan tambal gigi yang dipakai jutaan orang setiap hari. Siapa yang melakukan penelitian ini, mengapa, dan apa hasilnya? Itulah inti dari riset drg. Heribertus Dedy Kusuma Yulianto, M.Biotech., Ph.D., yang dilakukan di University of Groningen, Belanda, dan mendapat izin etik dari Komite Etik Penelitian Kedokteran Universitas Gadjah Mada, Yogyakarta. Hasilnya mengejutkan: bakteri penyebab karies gigi, <em>Streptococcus mutans<\/em>, ternyata lebih banyak berkerumun pada tambalan yang mengandung ikatan ester lebih tinggi \u2014 dan bakteri itu pula yang mempercepat kerusakan tambalan dari dalam rongga mulut.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Tambalan Gigi Bukan Sekadar Plastik Keras<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Resin komposit adalah bahan restorasi gigi yang kini paling sering digunakan untuk menambal kerusakan akibat karies. Bahan ini terdiri dari empat komponen utama: matriks polimer organik berbasis metakrilat, partikel pengisi anorganik (filler), agen pengikat seperti silan, serta sistem inisiator-akselerator untuk polimerisasi. Secara visual, tambalan ini terlihat kokoh dan berwarna menyerupai gigi asli. Namun di lingkungan mulut yang lembap, asam, dan penuh mikroorganisme, bahan ini terus-menerus mengalami tekanan degradasi.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Persoalannya bukan hanya soal abrasi mekanis. Biofilm \u2014 lapisan tipis komunitas bakteri yang menempel di setiap permukaan di dalam mulut, termasuk permukaan tambalan \u2014 mengandung enzim-enzim yang mampu memecah ikatan kimia dalam resin komposit. Ikatan ester, yang banyak ditemukan dalam monomer seperti Bis-GMA dan TEGDMA, dikenal rentan terhadap hidrolisis enzimatik. Pertanyaannya: bakteri mana yang paling bertanggung jawab, dan seberapa cepat kerusakan itu terjadi?<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Eksperimen di Dalam Mulut Manusia<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Untuk menjawab pertanyaan itu, Dedy Yulianto dan tim peneliti dari University of Groningen merancang studi <em>ex vivo<\/em> yang melibatkan 15 relawan sehat berusia 18\u201330 tahun. Masing-masing relawan menggunakan alat palatal \u2014 semacam pelat yang menempel di langit-langit mulut \u2014 yang di dalamnya tertanam dua keping sampel resin komposit berukuran kecil.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Dua jenis komposit dibandingkan: Beautifil II (Shofu Inc., Jepang), komposit direk nanohybrid dengan kandungan ikatan ester tinggi (HE), dan Lava Ultimate\/CAD (3M ESPE, AS), komposit indirek berbasis CAD\/CAM dengan kandungan ikatan ester rendah (LE). Kandungan ikatan ester masing-masing material diverifikasi menggunakan X-ray Photoelectron Spectroscopy (XPS) sebelum penelitian dimulai. Hasilnya: komposit HE memiliki sekitar 16% oksigen yang terlibat dalam ikatan ester, sementara komposit LE hanya 10%.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Selama dua periode masing-masing 30 hari, relawan dibagi menjadi dua kelompok: satu kelompok menyikat alat palatal dengan air dua kali sehari, kelompok lain tidak menyikat alat tersebut sama sekali. Setelah setiap periode, biofilm diangkat dari permukaan komposit, DNA diekstraksi, lalu dianalisis menggunakan teknik PCR-Denaturing Gradient Gel Electrophoresis (DGGE) untuk mengidentifikasi komposisi bakteri. Degradasi material diukur dari perubahan sudut kontak air permukaan komposit, yang memungkinkan perhitungan persentase paparan partikel filler sebagai indikator kerusakan matriks resin.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\"><em>S. mutans<\/em> dan Siklus Kerusakan yang Mempercepat Diri Sendiri<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Hasil analisis menunjukkan pola yang konsisten. Prevalensi <em>S. mutans<\/em> pada biofilm di permukaan komposit HE hampir dua kali lebih tinggi dibandingkan komposit LE \u2014 terutama pada kelompok yang tidak menyikat alat palatal. Bersamaan dengan itu, komposit HE mengalami degradasi yang jauh lebih besar, ditandai oleh peningkatan signifikan pada persentase paparan partikel filler di permukaannya.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Yang lebih mengungkapkan: ketika data dipilah berdasarkan ada atau tidaknya <em>S. mutans<\/em> dalam biofilm, komposit HE pada relawan yang biofilmnya mengandung <em>S. mutans<\/em> mengalami peningkatan paparan filler yang jauh lebih besar dibandingkan relawan tanpa <em>S. mutans<\/em>. Sementara komposit LE tidak menunjukkan peningkatan paparan filler yang signifikan, terlepas dari ada tidaknya <em>S. mutans<\/em>.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\"><em>&#8220;Ini adalah studi pertama yang menghubungkan prevalensi S. mutans yang lebih tinggi dalam biofilm klinis dengan degradasi komposit ber-ikatan ester tinggi. Enzim esterase yang diproduksi S. mutans terbukti bertanggung jawab atas degradasi komposit secara in vivo.&#8221; \u2014 drg. Heribertus Dedy Kusuma Yulianto, M.Biotech., Ph.D., dalam disertasinya di University of Groningen (2017)<\/em><\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Mekanismenya diduga berjalan seperti lingkaran: enzim salivari memulai degradasi awal permukaan komposit, partikel filler mulai terpapar, dan permukaan yang semakin kasar itu justru menarik lebih banyak <em>S. mutans<\/em> untuk menempel. Bakteri yang menempel kemudian memproduksi lebih banyak esterase, mempercepat pelepasan matriks resin, dan seterusnya. Sebuah siklus degradatif yang mempercepat dirinya sendiri.<\/p>\n\n\n\n<h2 class=\"wp-block-heading\">Sikat Gigi Masih Jadi Benteng Pertama<\/h2>\n\n\n\n<p class=\"wp-block-paragraph\">Temuan lain yang tidak kalah penting: penyikatan, bahkan hanya dengan air tanpa pasta gigi, secara bermakna memperlambat perkembangan biofilm degradatif ini. Prevalensi hampir semua strain bakteri lebih tinggi pada kelompok yang tidak menyikat alat palatal. Degradasi komposit HE memang tetap terjadi pada kedua kelompok, tetapi lebih parah pada kelompok tanpa penyikatan.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Ini bukan sekadar pengingat klinis biasa. Riset ini memberi dasar ilmiah yang kuat mengapa kebersihan mulut di sekitar restorasi komposit \u2014 membersihkan bukan hanya gigi, tapi juga permukaan tambalan itu sendiri \u2014 punya konsekuensi langsung terhadap umur panjang material restorasi.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Implikasi lainnya mengarah ke pengembangan material. Jika kandungan ikatan ester dalam matriks resin adalah faktor utama yang membuat komposit rentan terhadap serangan enzimatik bakteri, maka formulasi komposit masa depan yang meminimalkan ikatan ester \u2014 atau menggantikannya dengan ikatan kimia yang lebih tahan terhadap esterase \u2014 bisa menjadi jalur pengembangan yang menjanjikan.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Tambalan gigi mungkin terlihat diam dan pasif di dalam mulut. Tapi seperti yang ditunjukkan riset ini, ia hidup dalam ekosistem yang dinamis \u2014 dan bakteri di sekelilingnya tidak pernah berhenti bekerja.<\/p>\n\n\n\n<p class=\"wp-block-paragraph\">Penulis: drg.
